Report and investigate two depot incidents
You are the Occupational Health and Safety Coordinator.
What has happened
Two events at the Kellerton depot need to be handled properly. On Creek Road the grader clipped a marker post while turning on a narrow verge, with no injury but damage to the post and a scrape on the blade. At the Church Street drain pit a crew member leaned into the pit to clear leaves without a gas test, which is a confined space near miss. Both were reported verbally to Hemi Tauroa but the forms have not been completed. Under the Occupational Health and Safety Policy both must be in the Incident and Hazard Register within 24 hours. Hemi wants corrective actions agreed and a toolbox talk ready for Monday's pre-start.

Evidence guide
Forms must be factual, complete and free of speculation about blame. Check the notifiable decision is reasoned against the policy, that the investigation goes beyond the individual to procedure, equipment and supervision, and that corrective actions have owners and dates. The toolbox talk must be practical and specific to pit entry.
