What has happened
A raised concrete slab outside Marlow Flat Primary School has caused a child to trip. The slab has been ground and coned as a temporary measure and works order WO-26-0415 is open to replace three slabs before Friday. School drop-off runs from 8:15 am to 9:00 am and the footpath is the main route for walking students. Two houses share the footpath frontage. Last week a crew member strained his back lifting a slab without the vacuum lifter, and the manual handling refresher has not yet run. Hemi Tauroa has allocated the footpath truck and a two-person crew but the vacuum lifter is booked on another job on Wednesday.

The job safety analysis must identify the real hazards of the task and specify controls, including the vacuum lifter as mandatory. Check that the pedestrian route works during school peak times, that the notifications give the required notice, and that the works order shows the preparation steps completed before the status changes to Scheduled.
